Provider
Class PracticeAffiliation

This class represents information about the association between an Individual Provider and a Group Practice (or an individual Practice, if the provider is self-employed), or an Individual Provider to a Clinic.
A Practice is an Organizational Provider composed of one or more Provider for the purpose of providing medical services. When the Practice is composed of multiple Providers, it is often called a Group Practice.
A Clinic is "a place in which outpatients are given medical treatment or advice, often connected to a hospital, or a similar place staffed by physicians or surgeons specializing in one or more specific areas: eye clinic" - Collins English Dictionary.
Note that the FHIM makes a simplifying assumption that all Providers are associated with at least one Organizational Provider. Even when a physician is self-employed, we assume that the physician is incorporated, or has some kind of organizational face.
